precedes the selection of surgical technique), is synchronized with the
textbook and incorporates numerous improvements over the first edition. The
illustrations (drawn in the operating room during the actual procedure) are
designed to give a level of detail that requires little labeling and a sense of
realism that simulates the operative field; narrative description is intended
